Myasthenia Gravis: étiologie, physiopathologie & approches thérapeutiques

La myasthénie fait partie des maladies auto-immunes qui touchent plus de 5% de la population. Il s’agit de maladies multifactorielles qui mettent en jeu un terrain génétique et hormonal prédisposant, des anomalies du système immunitaire, et qui sont déclenchées par des facteurs encore non-identifiés. La myasthénie est due à des auto-anticorps dirigés contre des composants de la jonction neuromusculaire, principalement le récepteur de l’acétylcholine (RACh, 85% des cas) mais parfois aussi le récepteur tyrosine-kinase spécifique du muscle (MuSK) ou la protéine LRP4 interagissant avec l’agrine. Ces auto-anticorps diminuent l’efficacité de la transmission neuromusculaire et entrainent une fatigabilité anormale des muscles.

Le thymus est très probablement le site d’initiation de la myasthénie avec anticorps anti-RACh. En effet des anomalies histologiques du thymus sont très fréquentes : 50-60% des patients présentent une hyperplasie folliculaire avec centres germinatifs et 10-15% une tumeur du thymus. La thymectomie est l’un des traitements proposés à ces patients.

Les travaux de recherche développés par l’équipe ont pour objectifs de comprendre les mécanismes étiologiques et physiopathologiques impliqués dans la myasthénie, et de proposer de nouvelles thérapies. Plus spécifiquement, nos objectifs sont de :

  • Elucider les mécanismes étiologiques impliqués dans l’auto-immunité en analysant l’impact des hormones sexuelles et des perturbateurs endocriniens dans les processus de tolérance centrale.
  • Comprendre les mécanismes cellulaires et moléculaires à l’origine de l’inflammation et du remodelage thymique observé chez les patients.
  • Étudier les défauts immuno-régulateurs dans la myasthénie en étudiant le phénotype fonctionnel des cellules périphériques et thymiques par la cytométrie de masse (CyTOF).
  • Développer de nouvelles approches thérapeutiques. Dans ce contexte, nous étudions le potentiel immuno-modulateur et thérapeutique des cellules souches mésenchymateuses, et le potentiel de molécules interférant avec les voies inflammatoires.
  • Rechercher des biomarqueurs dans le sérum des patients pour suivre l’évolution de la maladie et la réponse aux divers traitements.
